Mission

Oakhurst Hospice embraces patients with any kind of life-limiting illness. We provide services and programs that can help people live complete and rich lives in spite of their loss, grief and terminal diagnoses. The palliative plan of care is formulated by the input from the patient, caregiver, patient’s physician and our medical director. Our nurses, social workers, home health aides, spiritual advisors and trained volunteers work together as an expert team to implement the palliative care plan. The team focuses on providing comfort and pain relief when a cure is not possible. The goal is to help each person live as fully as possible and to help the family and loved ones participate in their care.
